The Case Manager/Service Coordinator assists the prospective person to become a participant and coordinates, advocates and monitors the provision of all services in the Service Plan. Services may include Medicaid State Plan services, Non-Medicaid federal, state and locally funded services, as well as educational, vocational, social and medical services. The goal is to increase the participant's independence, productivity and integration into the community while maintaining the health and welfare of the individual.Bachelor's degree or associate degree with experience required. This is a Full-Time position starting at $45,000.00 with the ability for growth. This position is eligible for medical reimbursement.

Minimum Salary: 45,000 Maximum Salary: 50,000 Salary Unit: Yearly
